Important Health Office Reminder - March 2026
Yearly Student Physicals/Sports Eligibility
We want to remind all parents and guardians of the vital importance of submitting your son's yearly physical examination. To ensure your son’s eligibility for sports participation and maintaining his health and safety, it is critically important that the School Nurse receives a copy of your son's physical each time it is completed by their physician. Be sure to include a copy of the immunization record as well, each year. Please email all documents to nurse@sjci.com.
Why is the Yearly Physical Needed by the School Nurse?
Maintaining up-to-date health records is not only essential for your son's well-being but also a requirement for several school activities and state laws:
- Sports Participation: Per New York State law, every student must have a current physical, less than one year old from the start of the current sports season.
- State Mandates (9th and 11th Grade): New York State Education Law specifically requires mandated health examinations for students entering 9th and 11th grade, as well as for all new entrants to the district.
- Mandated Screenings: These physicals are also used to record state-mandated screenings, such as scoliosis checks (required for boys in 9th grade) and vision/hearing screenings (required for 11th grade).
- Annual Health Updates: Yearly physicals provide the school with necessary updates on your son's health status, ensuring we can provide the best care possible during the school day.
How to Submit the Physical
Please ensure the physical is documented on the Required NYS School Health Examination Form. While most doctors' offices use this format, we can only accept this specific state-mandated form. You may submit completed physicals using one of the following methods:
- Email: Send a digital copy to Mrs. Joanne Cascio, our School Nurse, at nurse@sjci.com.
- In-Person: Drop off a hard copy at the school's front office or mail a copy to St. Joe’s if needed.
Questions or Concerns?
If you have any questions regarding these requirements or need to check your son's status, please do not hesitate to contact Mrs. Joanne Cascio, RN, in our Health Office by email or by phone. Email: nurse@sjci.com, Phone: (716) 270-4128.

Report Absences: When calling the attendance line, please be specific about your child’s symptoms (e.g., fever, cough, or positive test result). This helps us track current health trends in our school.
-
Hand Hygiene: Encourage your child to wash their hands frequently with soap and water.
